A six week health education program to provide education regarding self-management techniques needed to assist client with chronic disease including diabetes, asthma, depression, cardio-vascular disease, hypertension, cancer. Statewide referrals accepted. Any adult with chronic disease (or their spouse/caregiver), including diabetes, cancer, depression, hypertension, and cardio-vascular disease.

The Diabetes Education Program, facilitated by a CDOE Certified Registered Nurse, Dietitian, and Pharmacist, provides 6-week sessions to patients with diabetes and their caregivers/significant other. This is a statewide program for individuals diagnosed with diabetes. The Tri-Town Health Center is an American Diabetes Association Recognized Site of Excellence for Diabetes Education and is also a RI Department of Health CDOE Certified Site.

Provides CONFIDENTIAL health visits for physicals, PAP smears, STD screening, and other reproductive-related issues. Provides free contraceptives including oral, Depo-Provera, and patches. Available to men and woman of reproductive age who are uninsured or underinsured.

Provides advocacy and assistance with applications for RIte Care and RIte Share. Provides pre-screening and assistance with Food Stamps, RI Medical Assistance, WIC, Head Start, Fuel Assistance, and Social Services. The Health Center monitors and tracks patient health disparities to improve health outcomes and submits the data to the State Registry in the areas of diabetes, pre-diabetes, colo-rectal screening, cardio-vascular and asthma.

Program is conducted by certified, bilingual Tobacco treatment Specialists using the Centers for Disease Control and Prevention mode, and adheres to the Standards of Practice prescribed by the Association for the Treatment of Tobacco Use and Dependence Services.

Provides monthly food vouchers to purchase nutritious food, including milk, eggs, cheese, iron fortified cereals, and juice. Monthly Nutrition Education and Counseling services are provided by Certified and Licensed Dietitians.
