Family Medicine Physician


Tri-CountyCommunity Action Agency is currently seeking full-time Family Medicine Physicianto provide primary care services in our Federally Qualified Health Center(FQHC).  Family Medicine Physicians will workcollaboratively with other team members in providing high-quality,comprehensive primary medical care to Tri-County patients of all ages, usingbest practice and evidence-based standards.Tri-County is an NCQA recognized Level III Patient Centered Medical Homeand, as part of the team-based approach to care, the physician will participatein coordinating care for patients, supporting patients and families in self-managementand engaging in continuous quality improvement activities aimed at specificpatient population needs.   We are particularly seeking individuals with astrong commitment to community health and working with the medicalunderserved. 


Qualifications:           Candidate must possess a license to practicemedicine in the State of Rhode Island and have a valid DEA license.  All physicians must becredentialed, or eligible for credentialing, with insurance companies as wellas through the National Practitioner Databank.Family MedicinePhysicians must be Board Eligible or Board Certified.  Prior clinical experience in primarycare, specifically working in a Federally Qualified Health Center (FQHC)environment, is a plus. Proficiency with electronic medicalrecords (EMR) is preferred. We particularly seek physicians with a commitmentto our mission “To provide accessible,quality, and comprehensive social, educational, health, prevention, and otherservices to people in need, while empowering them to achieve the highest possiblelevel of self-sufficiency and quality of life.” In our dynamicteam-based practice, excellent communication and interpersonal skills areessential. 


Tri-County Community Action Agency’sHealth Center is open Monday thru Thursday 8:30 am – 8:00 pm; Friday 8:30 am –5:00 pm providing Medical, Behavioral Health and Dental services.  Tri-County is scheduled to begin providingservices on Saturdays in the very near future.Physicians are required to work at least one evening per week and arerequired to participate in 24-hour on-call coverage on a rotatingschedule.  Hours are otherwise negotiableand flexible.

Tri-County Community Action Agency’sHealth Center is an NCQA Level III Patient Centered Medical Home that has beenproviding quality, comprehensive health care since 1972.  Our main Health Center facility is located inJohnston, Rhode Island and we recently expanded services, opening a new HealthCenter site in North Providence, Rhode Island.We are licensed by the RIDepartment of Health and participate in many of their Chronic Disease Preventionand Self-Management Programs.  We arealso licensed by the RI Department of Behavioral Health DevelopmentalDisabilities and Hospitals (BHDDH) to provide outpatient Substance Abuse andMental Health Counseling and Treatment Services.  Our Health Center provides a fully IntegratedBehavioral Health and Primary Care Program and is part of the HRSA-SAMHSAPartnership.  As part of a CommunityAction Agency, our patients benefit from the additional social servicesavailable from our Agency including WIC,, Early and Basic Head Start, FamilyCare Community Partnership, Youth Opportunity Center, Employment and TrainingPrograms; Youth Diversionary Program, Substance Abuse Prevention Coalitions, TeenPregnancy and Parenting Program; Financial Literacy Counseling, includingassistance in applying for Earned Income Tax Credit Credits , Emergency Foodand Shelter, Community Food Bank, SNAP, Home and Community Based CaseManagement Program for Elderly and Disabled Adults program, Fuel Assistance, Weatherization,Emergency Boiler Repair, Appliance Management Program  And Joyful Learning Child Care Academy forinfants, toddlers and preschoolers.
